HIV/AIDS CASE MANAGER
General Description of the Job Class
In accordance with the mission of West Coast AIDS Foundation, the HIV/AIDS Case Manager, under the direction of the Executive Director and Physicians, provides linkage, monitoring, planning and advocacy for the most appropriate HIV services for Adults infected with HIV/AIDS.
Duties and Responsibilities of this Level
- HIV/AIDS Case Management duties may include but not limited to the following:
- Conduct comprehensive assessments for people with HIV disease; develop, implement and revise service plans on an ongoing basis.
- Assist clients in accessing needed medical care, dental care, substance abuse services and mental health services.
- Accessibility to regular and reliable transportation to travel extensively throughout Pinellas County.
- Documenting all services through the established record keeping policies and procedures.
- Responds to calls from program clients within 24 hours.
- Attend and complete mandatory trainings.
- Provides assistance with the acquisition of public assistance, Medicaid, food stamps, Social Security and other financial benefits and entitlements.
- Work collaboratively with other AIDS Service Organizations, as well as Mental Health and Chemical Dependency Programs to ensure adequate care for clients.
- Attend and participates in team and all staff meetings.
- Ability to organize, prioritize multiple projects and meet deadlines in a time-sensitive environment.
- Ability to develop good working relationships with internal and external customers.
- Demonstrate ability to work with people of diverse socio-economic backgrounds, ethnicities, sexual orientation and lifestyle.
- Maintains confidentiality regarding patient and business within the office
- Be intimately familiar with appropriate sources of referral for home care services, mental health and chemical dependency services, financial, housing, food, emotional support, etc., for people with HIV/AIDS.
- Familiarity with Ryan White Care Act Title I, II, and III and program requirements.
- Previous experience in the entry of patient data, including but not limited to demographic, clinical and other relative tracking information.
Required Qualifications at this Level
Education: Bachelor’s degree from accredited college or University.
Experience: Previous experience in case managing individuals
Degrees, Licensure, and/or Certification: BA Degree in Social Work or closely related field.
Knowledge, Skills, and Abilities:
- Excellent communication skills
- Good customer service skills.
- Possess Positive attitude/behavior.
- Proficiency in Microsoft Office programs, including Word and Excel.
- Ability to respond quickly to change.
- Ability to keep required documents legible and organized in client record for monitoring purposes.
